Medical Forms

If you child has a medical condition that the school needs to be aware of, please download the appropriate management and emergency treatment plan below. These plans should be completed with your child's doctor at the beginning of each school year or prior to enrolment. If any changes are made to your child's treatment plan throughout the year, please make sure that the school receives a updated copy of the treatment plan.
